A standard form requesting subscriber details is completed by the investigating officer. This form includes information known to the investigator which is sufficient to allow the ISP to identify the requested information. For instance, the investigator may know a name and seek a corresponding IP address, or vice-versa. The form is then authorised by a senior officer and sent to the relevant ISP. The results are then received back by the investigating officer.

Law enforcement and service provider(s) have developed protocol for requesting and delivering information. This can be done by sending a preservation letter to the service provider followed by a court order for its release. It is important that voluntary requests are done with appropriate personnel.
